The Total Nose Approach™

Breathe Beautifully!

The Total Nose Approach™, a concept that our physicians coined over three decades ago, derives from the ability to take care of even the most severe functional issues of the nose and sinuses in minimally invasive fashion, so that the aesthetic aspects of the nose can be addressed if necessary at the same time. So often the two go hand in hand.

Physicians Dr. Nachlas and Dr. Hancock at the Nose and Sinus Institute of Boca Raton have the absolute most experience with the Total Nose Approach™ in the world.

Frequently Asked Questions

The Nose and Sinus Institute of Boca Raton pioneered the Total Nose Approach™ to sinonasal surgery more than three decades ago, when, for the first time, minimally invasive sinus surgery was performed at the same time as rhinoplasty surgery. The Total Nose Approach™ to surgery means that all of your functional sinonasal complaints and cosmetic concerns can be addressed during one surgery, with one anesthetic, and one recovery.

The Total Nose Approach™ to the nose and sinuses is a philosophy developed by Dr. Nathan E. Nachlas three decades ago. Dr. Nachlas was the Chief Resident in the Department of Otolaryngology-Head and Neck Surgery at the Johns Hopkins Hospital where minimally invasive sinus surgery was first introduced in 1985. Minimally invasive sinus surgery enabled surgeons to treat a variety of serious functional issues with the nose and sinuses using telescopes and microscopic instruments – for the first time, no external incisions were required! After his chief residency year, Dr. Nachlas went on to complete a Facial Plastic and Reconstructive Surgery Fellowship in Beverly Hills, California. There, he acquired expertise and artistry in restructuring the outside of the nose. With these two former disparate disciplines completed, he started his practice in Boca Raton, where he developed the Total Nose Approach™.

Since its conception, Dr. Nachlas has treated thousands of satisfied patients with his Total Nose Approach™. Dr. Melyssa Hancock joined the Nose and Sinus Institute with extensive training in both the functional and cosmetic treatment of the nose and sinuses and uses it to build on Dr. Nachlas’ decades of experience.

Both Dr. Nachlas and Dr. Hancock are experts in the treatment of functional and aesthetic nasal issues and help patients attain a nose that is cosmetically beautiful, as well as functionally sound.

The “Total Nose Approach™” is a philosophy, not a procedure. The surgery in which the inside of the nose, as well as the outside of it, is optimized is known as the “Total Nose Approach™ Procedure”. A Total Nose Approach™ procedure may involve sinus, septum, and/or turbinate procedures to address the internal issues, as well as a Rhinoplasty procedure to address the outside of the nose.

A Total Nose Approach™ procedure may be for you if you have any of the following symptoms or concerns:

  • Crooked nose
  • Nasal hump/bump
  • Blocked nose
  • Difficulty breathing
  • Headaches
  • Sinus congestion or pressure
  • Loss of the sense of smell
  • Fatigue

Any or all of these symptoms can stem from problems with the outside or inside of your nose. The nose is a complex organ; many people assume that the nose is simply an external feature of the face that has a variable size and contains two nostrils, a tip, and a bridge. The nose is so more than that! It actually extends from that visible tip, all the way back toward the center of the head. The inside of the nose is intricately designed with multiple different structures and spaces that are responsible for nasal breathing (nasal septum, nasal valves, and sinuses), air filtration and humidification (turbinates), and transportation of air through its many complexities to the lower airway (lungs). The internal nose also has protective functions, which can sometimes go awry, leading to sinus infections, headaches, congestion, excessive drainage, problems with smell and taste, and allergies, among many other issues.

Changing the shape of the nose can enhance your appearance, give you confidence, and improve your breathing at the same time. At the Nose and Sinus Institute of Boca Raton, Dr. Nachlas and Dr. Hancock perform Rhinoplasty for patients who would like to reshape, reduce, or augment their noses for both cosmetic and functional reasons. Patients of all ages and ethnicities seek out their highly specialized training to improve both the form and the function of their noses.
