The Nose and Sinus Institute of Boca Raton pioneered the Total Nose Approach™ to sinonasal surgery more than three decades ago, when, for the first time, minimally invasive sinus surgery was performed at the same time as rhinoplasty surgery. The Total Nose Approach™ to the nose and sinuses is a philosophy developed by Dr. Nathan E. Nachlas three decades ago. Dr. Nachlas was the Chief Resident in the Department of Otolaryngology-Head and Neck Surgery at the Johns Hopkins Hospital where minimally invasive sinus surgery was first introduced in 1985. Minimally invasive sinus surgery enabled surgeons to treat a variety of serious functional issues with the nose and sinuses using telescopes and microscopic instruments – for the first time, no external incisions were required! After his chief residency year, Dr. Nachlas went on to complete a Facial Plastic and Reconstructive Surgery Fellowship in Beverly Hills, California. There, he acquired expertise and artistry in restructuring the outside of the nose. With these two former disparate disciplines completed, he started his practice in Boca Raton, where he developed the Total Nose Approach™.

Since its conception, Dr. Nachlas has treated thousands of satisfied patients with his Total Nose Approach™. Dr. Melyssa Hancock joined the Nose and Sinus Institute with extensive training in both the functional and cosmetic treatment of the nose and sinuses and uses it to build on Dr. Nachlas’ decades of experience.

The “Total Nose Approach™” is a philosophy, not a procedure. The surgery in which the inside of the nose, as well as the outside of it, is optimized is known as the “Total Nose Approach™ Procedure”. A Total Nose Approach™ procedure may involve sinus, septum, and/or turbinate procedures to address the internal issues, as well as a Rhinoplasty procedure to address the outside of the nose.
